Epilog’s Transition Specialists offer professional, emotional, practical, social, physical, and spiritual support to ensure end-of-life needs are met. Several of our team members have earned certificates as “End-of-life doulas”. We can assist with “unfinished business” and begin preparation of a unique and meaningful legacy. Our services help take the pressure off family and friends so they can focus time and attention on the person they love – just as it should be.

Our team will handle all non-medical aspects of care and comfort – we are someone to talk to about the things you don’t want to talk to others about, validate concerns and fears, align with beliefs and values, advocate and support your journey as desired.
Here for Your Family and Friends
We will assist with the fear of dealing with anticipatory grief of friends and family, and address emotionally charged relationships with integrity and confidentially – even the tough conversations.
Support
We will offer support in preparation of amazing and unique legacy plans – including, for example, music, art, photos, letters, and poetry. Our team will support clients, friends and family throughout the end-of-life process – from the beginning, all the way to sitting vigil. With access to a wide range of resources to help as needed, all while keeping everyone’s comfort as our primary focus.
Preparation
Our team will ensure necessary documents are prepared – Celebration of Life wishes, advance directives, power of attorney, and will. We’ll arrange other support services as needed (cleaning, meal delivery, etc.), and support plans for home funerals if desired,
Working Together
We can collaborate with others (nursing, palliative care, hospice, etc.) to ensure there are no gaps in service/care. And later, we can arrange funeral or Celebration of Life plans – at home or at another location of your choice.
Education
We will work with all involved to help demystify the end of life process. This includes having those tough conversations with any children involved.
